Office Policies

All new cosmetic patients or established patients seeking new treatment are required to have a consultation prior to receiving treatment. Our consultation fee is $200, our customized laser and/or skin care consultation fee is $50.

The consultation fee is due at the time of scheduling your consultation. The consultation fee is non-refundable if the appointment is canceled or the patient is “no shown” for the scheduled appointment.

Additional fees or deposits may be charged for repeat late cancellations and “no shows” for scheduled appointments.

Children under the age of 16 are discouraged from accompanying adults who are receiving treatment.

All clients interested in laser procedures or chemical peels are required to consult with a provider before any procedures are performed. Unfortunately, laser procedures and chemical peels are not appropriate treatments for all skin types or certain skin conditions. Our staff will perform an in-depth consultation at the time of your consultation to determine if you are a candidate for the desired treatment.

Patient Forms

Patients can download and print the forms at home, and by completing them in advance, you’ll save time during your visit. Registering and checking in ahead of time will also help make your appointment more efficient.

Please have this information ready, along with your current medical insurance details, when you visit our office. If you have any questions while filling out the forms, just do your best—our staff will assist you on the day of your visit.

Return Policy

Michael J Orseck, MD will gladly accept the return of all retail products for an office credit or exchange only. All products must be new, unused, and contain the original packaging as well as the receipt dated within 7 days of purchase. No cash refunds. Exchanged products cannot be returned for exchange or refund. Policy is subject to change.
